COMPLIANCE
Before you can get involved with volunteer activities at OSM, you MUST be compliant with the Archdiocese of Chicago Volunteer Requirements.
Archdiocese of Chicago Volunteer Compliance Requirements:
The Archdiocese of Chicago has mandated requirements for all volunteers who work with children in Archdiocesan schools. All requirements must be completed prior to volunteering with children at Old St. Mary’s School.
Appropriate forms or instructions for all the items are on the Archdiocese of Chicago website. Please review the instructions and forms found below for Returning Parents and New Parents.
Please return all forms to the school office. If you have any questions please email fsa@osmschool.com.
*For non-compliant volunteers, other opportunities are available that do not include working with children.
Returning Parents
If you were compliant last year, to remain compliant, you need to complete the following:

Acknowledgement of Mandated Reporter Status (CANTS 22 & CANTS 689) Form - Please sign and return to pmurphy@osmschool.com. This must be completed annually.

Illinois Mandated Reporter Training - The training consists of a pre-test, a 60-90 minute self-paced training, and then a post-test. Once completed you can prompt the system to send your certificate directly to pmurphy@osmschool.com or you can download and email it. This training will need to be completed every three years.

The 2023 Standards and Behavior - Please log into your VIRTUS account to e-sign. This must be completed annually.

Returning parents who are unable to access your account: If your VIRTUS account is inactive, please email pmurphy@osmschool.com to have the Archdiocese activate your account.
New Parents

Create a Virtus Account

Create an account for yourself at www.virtus.org (This is individual, NOT family)

Select Site in the Primary Location Box: Old St. Mary’s School (not, Old St. Mary’s, that’s the Parish)

Application Type: Volunteer

Complete an Online Criminal Background Check through your account

Shortly after completing the background check information on Virtus, look for an email from VERIFY2 and complete the information. It will be sent to the email account you put on your Virtus account. You must respond to this email to complete your background check

Add two references, one personal and one professional.

Parents must complete the following trainings after they set up their account:

Protecting God's Children - You will be prompted to complete this training online through your VIRTUS account. For more information please click here.

Please note, that a new account will have to be established for Mandated Reporter Training. This is separate from VIRTUS.

Please note, these forms MUST BE signed by hand. They cannot be electronically signed. They can be emailed to pmurphy@osmschool.com or handed in to the front desk of the school. These forms are filled out at the beginning of each school year (for new and returning parents) before you are eligible to volunteer with children.

All certifications for VIRTUS and Mandated Reporter Training needs to be sent to Patti Murphy, at pmurphy@osmschool.com. Certifications will be kept on hand at school.
